Advanced Pharmacist

Weve made exciting progress with our virtual ward and patient numbers are continuing to increase in line with our plan to reach 250 patients by the end of the year. We already have a team of experienced clinicians supporting the virtual ward and we are now want add pharmacists to our team to ensure they have access to medicines experts.

In this role you will be responsible for the provision of comprehensive clinical pharmacy services to virtual wards held by Shropshire Community Health NHS Trust. You will be embedded in the multidisciplinary team providing care to respiratory, cardiology and frailty patients in the virtual wards. In addition, you will support further development of clinical and pharmaceutical pathways. Antibiotics will often play an important role for our pathways, and you will be key in ensuring the antimicrobial stewardship is appropriately managed. You will be required to attend the Systems Antimicrobial Stewardship Group as a pharmacy representative.
