Would you like to work with patients in the place they call home to manage their medicines. The Pharmacy Virtual Ward team work with patients to facilitate early supported discharge or admission avoidance, conducting clinical medicine reviews and carrying out interventions where appropriate to improve pharmaceutical care. They will provide medication education, assess adherence and follow up patients as appropriate to monitor outcomes. The team work alongside multi-professional colleagues to improve the care of patients through promoting evidence based and patient-centered care. You will work as part of a multidisciplinary team to enable patients to manage their own medicines safely.
This is a rare and exciting opportunity for a highly motivated Pharmacy Technician to join our friendly Virtual Ward as a Specialist Pharmacy Technician. This is a brand new post (band 6, subject to evaluation) and we very much look forward to welcoming the successful applicant to our team.
You must be a registered Pharmacy Technician with the General Pharmaceutical Council and have significant post-qualification experience.
You must be adaptable to the needs of the Pharmacy Service, which may involve cross site and flexible working to support efficient delivery of the service.
