Virtual wards allow patients to receive the care they need at home safely and conveniently, rather than being in hospital.

This is an exciting opportunity to join a rapidly developing service where acutely unwell patients can be cared for in their own homes using technology devices for us to be able to monitor our patients.

This is a new service where nurses will identify acutely unwell patients on the wards who need hospital care but can be supported in their own homes. You will work with the multi- disciplinary to provide outstanding care to your patients and their families.

Working in a caring, supportive and professional environment you will be able to provide nursing care to our patients in the community virtually and face to face. This role will make a positive difference to people during their hospital admission. Using excellent nursing skills with outstanding interpersonal skills, endurance and empathy you will make both staff and patients feel cared for and supported. You will provide attention to detail, be a problem solver and care for our patients and families. You will be an integral part of the multi-disciplinary team

You will provide care to patients with complex and specialist needs and will work alongside the multi- disciplinary team to develop, implement and evaluate care plans.

You will assess the health and psychosocial needs of the patients within the hospital and community settings, you will plan and implement treatment, and use a wide range of services to meet their needs.

You will use a wide range of clinical skills to support the ongoing management of patients in their home environment.

You will identify patients suitable for virtual ward monitoring and be proficient in fitting the appropriate technology following training.
You will demonstrate effective communication, networking and liaison skills, ensuring high quality outcomes for the service providers and users.
